Practitioner directory: name, phone number, email, area of expertise, professional qualifications including related documents and regulatory bodies if applicable.
Volunteers: name, phone number, email, area of expertise, skills & any other information you provide.

Most of the personal information we process is provided to us directly by you for one of the following reasons:
Via a practitioner directory, volunteer database and newsletter.
To disseminate information from Create My Health CIC (CMH) – for example to add your details to our practitioner directory for potential clients to view –, to notify you of volunteer roles and update you about CMH via our newsletter.
We will use the information that you have given us in order to carry out the above activities.
We will not collect any information about you unless you choose to provide us with personal information, and we will only maintain this information for as long as required to fulfil the stated purpose.
We only keep information for as long as you are happy to share it and we will not disclose, sell, give or transfer any personal information without your prior consent or unless required for law enforcement of statute.
